Whats the highest it will elevate the bottom of a 21 inch monitor?  I want to raise the monitor above a 27 inch monitor.

It won't do that.  I can't answer exactly but, I KNOW it won't do that.  It isn't a bad stand.  I bought it because it states it supports up to a 32" monitor.  My goal was to put a 32" monitor in portrait mode.  It would NOT support the weight of my brand new Lenovo 32" monitor.  I tightened the screws as tight as they tighten, which broke the enclosed Allen wrench.  I had my own.  I used it and tightened as tight as it would go and it still would not support it.  It did "work" as I turned it into portrait mode and just let the weight of the monitor pull it down and it rests on the stand.  It did well but was a little "tippy".  I went to Home Depot and purchased a large clamp and went behind the monitor and clamed it to the desk.  This is my experience.  I cannot speak to what your experience would be.  While it states that it will support a 32" monitor, my experience leaves that in question.  Would it support it in normal landscape mode?  "Maybe" but, the manufacturer included one of those stands.  I didn't try it in that orientation because I didn't need it in that orientation.  Your mileage may vary.  It is not a "bad" product.  I personally think they should market it to max out at 27" instead of 32".  Again, this "my experience", your mileage will vary.  I can tell you though, it is NOT tall enough to do what you want to do.

Hi, you could buy a VESA adapter to hold the monitor to the mount.

Once I installed the monitor to portrait, how can I rotate it to vertical without un-mounting the monitor?

This is very much a manual process on this mount. any vertical pitch, vertical up and down needs to be tightened down with an allen wrench (which is provided).  If you are looking to mount in portrait. You would have to unmout the screws and remount vertically.  It is solidly built but better for a semi-permenant location vs frequently changed.  The only easy adjustment is that once it mounts and can swing left to right easily as that is the only articulated arm that does not have to be tightened down.

I wouldn't.  imacs weigh more than a regular monitor.  It's ok as far as monitor stands go but it will slip out of position easily.  I wouldn't use it on an imac.
